Insect bite hypersensitivity (IBH) also called summer eczema, is the most
frequent allergic dermatitis of horses and it can affect all horse breeds. This IgEmediated
dermatitis is caused by bites of insects of the genus Culicoides.
Dysregulation of the T-helper 2 (Th2)/regulatory T cell (Treg) response to
allergens is one of the main features of this disease. IBH is found worldwide and
is a notable welfare issue. It does not occur in Iceland because the causative
Culicoides are not native to Iceland, but the incidence of IBH is very high (≥50%)
in horses of the Icelandic breed imported as adults to Europe, the USA or other
regions where Culicoides are present. IBH-affected horses suffer from severe
itching, resulting in skin lesions following scratching and secondary infections.
